Eldesia Granger, MD, MPH
Dr. Eldesia Granger is a physician executive and health policy leader who has shaped
national programs focused on disease prevention, health promotion, health care
delivery and population health. As a principal consultant at THE 5Ps, LLC, a Service-
Disabled Veteran-Owned Small Business health care consultancy, she focuses on
business development and strategy, bridging health policy, digital health and clinical
care.
Previously, she served as a Medical Officer and Acting Deputy Director for the State &
Population Health Group at the Center for Medicare and Medicaid Innovation (CMS
Innovation Center). In this role, she partnered with states and clinicians on value-based
payment and care delivery models designed to drive innovation and advance health
care outcomes, quality, patient experience and affordability. At the CMS Innovation
Center, her work included the Medicare Diabetes Prevention Program, an evidence-
based lifestyle change program for Medicare beneficiaries with prediabetes that aims to
reduce the risk of type 2 diabetes.
Before joining the CMS Innovation Center, Dr. Granger supported federally funded
research and development centers. Her work included developing and implementing
policy solutions, conducting data ethics research, and collaborating with technical teams
to design health technologies. These technologies ranged from synthetic patient
medical records and clinical decision support systems to platforms for modeling and
simulation of health care delivery.
In her clinical practice, Dr. Granger is dedicated to helping people living with diabetes
and other chronic conditions achieve their optimal health using the pillars of lifestyle
medicine. She is board-certified in internal medicine, lifestyle medicine, pediatrics and
preventive medicine, and holds a Type 2 Diabetes Remission Certificate from the
American College of Lifestyle Medicine. Dr. Granger has been living with latent
autoimmune diabetes in adults (LADA) since 2019.
