CONSENT
I wish to provide to Beyond Type 1 photographs and information about myself (or my child), to promote Beyond Type 1’s mission to improve the lives of people living with type 1 diabetes today, and funding research for a cure tomorrow.

I grant to Beyond Type 1 (and Beyond Type 1’s assigns, licensees and successors) the irrevocable right to use the materials and information that I submit to Beyond Type 1 (such as my image, likeness, voice and/or interview or biographical material) in whole or in part, individually or together with other materials, in all forms and all media, including composite or modified representations, for all purposes, throughout the world and in perpetuity.
I waive the right to inspect or approve such materials created and/or used by or on behalf of Beyond Type 1 pursuant to this consent.
